Confidentiality & Notice of Privacy Policy

All information disclosed within sessions and the written records pertaining to those sessions are confidential and may not be revealed without your written permission, except where disclosure is required by law.   

As a Licensed Clinical Social Worker of the State Board of Licensed Social Workers, Megan Lammers will abide by its Code of Ethics.  You have the following rights as a client of the Oregon Board of Licensed Clinical Social Workers:

  • To expect that the licensed clinical social worker has met the minimal qualifications of training and experience required by state law;

  • To examine public records maintained by the Board and to have the Board confirm credentials of a licensee;

  • To obtain a copy of the Code of Ethics;

  • To report any complaints to the board;

  • To be informed of the cost of professional services before receiving the services;

  • To be assured of privacy and confidentiality while receiving services as defined by rule and law, except for the following exceptions:

    • When there is reasonable suspicion of child, dependent, or elder abuse or neglect.

    • When you make known any intention to harm yourself or seriously injure another

    • When information that would facilitate treatment of a medical emergency

    • When court ordered to provide records or testimony in a case brought by you or against you

    • When submitting for payment by your insurance company or facilitate payment

    • Providing information concerning the therapist’s consultation or supervision;

    • Defending claims brought by client against therapist

Megan Lammers does not provide custody evaluation recommendations,  medication or prescription recommendations, medical advice, legal advice, fitness for employment reviews, as these activities fall outside my scope of practice.

Oregon law and the standards of the social work profession require that Megan Lammers will keep treatment records for at least 7 years and your right for your records to be safeguarded from unlawful use or disclosure. Please note that clinically relevant information from emails, text and faxes are part of the clinical records. Clients have a right to review or receive a summary of any records at any time, except in limited legal or emergency circumstances or when Megan Lammers assesses that releasing such information might be harmful in any way. 

Generally, parents of adolescents 14 and older in Oregon, or 13 and older in Washington, may see their adolescent’s records only with the written consent and permission of the adolescent.

Mental health therapists often have an influential position with respect to clients, and should avoid exploiting the trust and dependency of such persons. Sexual intimacy between therapist and client is prohibited. Megan Lammers will make every effort to avoid conditions and multiple (dual) relationships with clients that could impair my professional judgment or increase the risk of exploitation. Such relationships include, but are not limited to, business or close personal relationships with a client. When the risk of impairment or exploitation exists due to conditions or multiple roles, we will take appropriate precaution. Not all dual relationships can be avoided. However, Megan Lammers will never acknowledge working with you to others without your written permission. 

Megan Lammers does not accept friend or contact requests from current or former clients on any social networking sites (Facebook, Instagram, LinkedIn, etc). Megan Lammers believes that adding clients as friends or contacts on these sites can compromise your confidentiality and our respective privacy. Megan Lammers does keep a Facebook Page and an Instagram account for her professional practice.
